<p>What I love most about this 1906 ad from <strong>LD Caulk</strong> (now part of <a href="http://www.caulk.com/" target="_blank">Dentsply</a>) is how it trash-talks the competition while at the same time not really quantifying why their amalgamated alloy is better. Theirs is better <em>why</em>? Because they say so. Because they have &#8220;delicate laboratory instruments&#8221; and &#8220;full scientific equipment.</p>
<p>One other thing that&#8217;s interesting, try looking at Google Maps to <a href="http://maps.google.com/maps/place?ftid=0x89c6c62fa4be04c5:0xbc78826897f91174&amp;q=SE+corner+broad+chestnut+philadelphia+pa&amp;hl=en&amp;ved=0CA4Q-gswAA&amp;sa=X&amp;ei=GWEbTr7OD4XOMoLtkN8G" target="_blank">see what is now built</a> at the SE corner of Broad and Chestnut Streets in Philly. That&#8217;s some prime real estate, one block from Penn Square downtown.</p>

<p>Apparently their product was worthy enough to help Caulk amass a small fortune and survive until the present day so we can&#8217;t totally dismiss the claims as bogus. Then again, alloy itself has made it into the 21st century, but not without a fight, what with mercury fears and the demand for tooth-colored restorations.</p>
<p>The next time you see a 50-year old amalgam in a patient&#8217;s mouth, you can thank turn-of-the-century tech for recognizing that expansion and contraction have always been the bane of alloys, and that filling you&#8217;re seeing is proof of Caulk&#8217;s &#8220;knowledge and accuracy&#8221; 50 years later. So 100 years of success- that ain&#8217;t a bad track record.<br />
And one more takeaway? Carpet-bagging still exists, only today when you &#8220;get what you pay for&#8221; we call it dabbling in the grey market.<br />

<p>Yesterday on his network television show <a href="http://newsfuzion.com/2011/04/15/dr-oz-confronts-critics-of-thyroid-protection/" target="_blank">Dr. Mehmet Oz defended his remarks</a> back in September about the necessity of lead thyroid collar use during dental x-rays whenever possible by stating that &#8220;there&#8217;s no good reason not to protect yourself against radiation.&#8221; While this is true, the bottom line is about using radiation in a way that leads to best outcomes in our patients, a point that several guest physicians also tried to make despite the protests of Dr. Oz.</p>
<p><a href="http://www.dentalbuzz.com.php5-20.dfw1-1.websitetestlink.com/wp-content/uploads/2011/04/xkcdradiation.pdf"><img class="alignleft size-full wp-image-1472" title="Dosechartradiation" src="http://www.dentalbuzz.com.php5-20.dfw1-1.websitetestlink.com/wp-content/uploads/2011/04/Dosechartradiation.png" alt="" width="153" height="178" /></a>Seriously, when you look at the sources of radiation in the world today, why worry about the miniscule intentional amounts? All radiation is dangerous, and it should be limited as much as possible, but the only way to avoid it totally is to dig a radon-free hole somewhere and live like a makedd roll gnat the rest of your life (ummmm. I mean <strong>naked mole rat</strong>. So hard to get that one right!). Calm your fears with this Radiation Dose Chart that was recently compiled, complete with facts about the Fukushima Nuclear plant (click on the image for full-size).</p>
<p>Another thing that I whipped up <a href="http://www.dentalbuzz.com/wp-content/uploads/2010/08/dental-radiation-doses.pdf" target="_blank">here</a> for all you DentalBuzz readers is a <a href="http://www.dentalbuzz.com/wp-content/uploads/2010/08/dental-radiation-doses.pdf" target="_blank">Medical Radiation Dose Chart</a> to download and show to your patients that have no idea how much radiation that they may be getting from the more common diagnostic procedures. I didn&#8217;t know a lot of it myself, so originally I created it to use in our office but you may find it helpful too. Did you know that it takes 10,000 PA radiographs to reach the <em>annual</em> limit of x-radiation? How many CT scans does it take to reach the limit? I&#8217;m too lazy to do all the math but it looks like 1 CT scan equals about 2,000 bitewings (film, not digital) according to this chart.</p>
<p>Regardless, we all have a responsibility to admit that we don&#8217;t know what we don&#8217;t know. And perhaps Dr. Oz may be a little right; we should do our best to protect our patients from radiation whenever possible, adapting the ALARA (As Low As Reasonably Achievable) principle with regards to radiation in dentistry.</p>
<p>Enter the media blitz.<a href="http://www.idixray.com/" target="_blank"> Interactive Digital Imaging</a> (IDI) is re-introducing its rectangular collimator with a PR campaign targeting consumers. Dental patients will be encouraged to &#8220;Look for the Green Ring&#8221; at their dental offices, similar to the &#8220;Ask Your Doctor About&#8221; style of direct-to-consumer advertising used by the<a href="http://www.idixray.com/" target="_blank"><img class="alignright size-full wp-image-1537" title="trualign" src="http://www.dentalbuzz.com.php5-20.dfw1-1.websitetestlink.com/wp-content/uploads/2011/04/trualign.png" alt="" width="200" height="181" /></a> pharmaceutical industry. Originally priced at $1295, the newly redesigned Tru-Align collimator will be offered at $795 with improvements that overcome cone-cut errors and enhancements that make the positioner more user-friendly.</p>
<p>My favorite feature of the <a href="http://www.idixray.com/videos/" target="_blank">Tru-Align system</a> is not that it can be used with any typical XCP holders (but still a good thing &#8211; this includes sensors, phosphor plates, and film), but that when you bump up the xray cone to the collimation ring, you get a magnetized lock-on and an audible beep to let you know that you will NOT be getting a cone-cut on that image. BAM! ZAP! If nothing else, the precision of this thing should impress our patients. And if they&#8217;re the kind that are really concerned about their radiation exposure, you can spew some factoids about reducing the absorbed dose from around 150 microsieverts to 30 microsieverts for an 18-image FMX series.</p>
<p>What it finally comes down to is having the right things to say to your patients when they ask about radiation, and addressing any real problems that you may be having in your office, like the assistant who has to re-take a periapical x-ray three times in order to get the one that you need. Or making sure that your thyroid collars are actually being Velcroed behind your patients&#8217; necks. Or halting the bacteria-fest breeding on your lead aprons. My bet is that&#8217;s the biggest offense of all. Ahhh, a problem for another day&#8230;..</p>

<p>First brought to market in 2005, <a href="http://www.isolitesystems.com/" target="_blank">Isolite™ Systems</a> originated the idea of combining dryfield illumination and isolation in a patented product called the Isolite, which costs about $1700 per operatory to set up. Whoa, steeeeep. The tubing and LED Smart Stick sure isn&#8217;t their loss leader, now, is it? Then you <em>still</em> have to buy a <a href="http://www.isolitesystems.com/shop/product.asp?id=26&amp;image.x=57&amp;image.y=8&amp;image=Select+Sizes" target="_blank">$2.50 mouthpiece</a> for each patient because they&#8217;re not sterilizable. All of it together is still easier and less expensive than your assistant.</p>
<p>But what if you want to keep your assistant, even though she kicks you under the chair all those times you say something stupid to a patient or start getting all OCD over a procedure? You&#8217;ve already ditched your overhead lamp for loupe-mounted headlights, so maybe you don&#8217;t even need the Lite part of the Isolite.</p>
<p>About a year ago, Isolite™ Systems indroduced a non-illuminating version called the IsoDry that runs a little less than half the cost of the original product. Both systems come with extra tubing.<em> Tooobing.</em> Makes me want to laze down the Guadalupe River with a six-pack in a styrofoam cooler. Don&#8217;t we have enough tubes and hoses to twist around each other already?</p>
<p>This brings us to the essence, the soul, the very magic of what makes the Isolite System the game-changer that it has become. It&#8217;s all about the <a href="http://www.isolitesystems.com/mouthpiece.asp" target="_blank">mouthpiece</a>. This transparent, comfortable, easy-to-insert soft piece of silicone not only attaches to high-speed suction to create a dry field, it replaces bite blocks, throat packs, cotton rolls, drying angles, and everything else that you used to cram in your patient&#8217;s mouth to create a perfect restorative environment.</p>
<p><a href="http://www.facebook.com/konaadapter" target="_blank"><img class="alignleft size-full wp-image-1411" title="isodryadapter" src="http://www.dentalbuzz.com.php5-20.dfw1-1.websitetestlink.com/wp-content/uploads/2011/04/isodryadapter.png" alt="" width="190" height="311" /></a>But in order to use the mouthpiece, the rule is, you have to buy an Isolite or an Isodry. You can&#8217;t just stick it onto your high-speed suction and use it solo.</p>
<p>Or <em>can</em> you?</p>
<p><a href="http://dentalsuction.wordpress.com/about/" target="_blank">Mark Frias, RDH</a>, can hook you up to go commando. Literally. He&#8217;s invented a hook-up mod for the Isolite mouthpieces that must have been driven by the frustration of trying to keep a squirmy six year old&#8217;s teeth dry for sealants with traditional isolation. You can see the differences between his design and the original Isolite on the left. It&#8217;s not sleek and sexy, but from a cost perspective this little adapter is a no-brainer. Mark calls it the <a href="http://KonaAdapter.wordpress.com/" target="_blank">Kona Adapter</a>. Why? Is he an Ironman Triathlete from Hawaii? Actually, I think he named it after his dog.</p>
<p>Whatever the case, the ingenuity here is striking at a great moment. Mark is having difficulty keeping Kona Adapters in stock if that&#8217;s any indication of its demand.</p>
<p>For those who are concerned about taking business away from Isolite, Mark suggests purchasing a single system for one of your operatories and <a href="http://www.facebook.com/konaadapter?sk=app_4949752878" target="_blank">fitting the rest with Kona Adapters</a>. This will give you the privilege of being an official Isolite customer so that you can be assured that you aren&#8217;t buying mouthpieces on the down low.</p>
<p>And everyone really wins here. Isolite could <em>give</em> the system away like Gillette gave away razor handles to sell you the blades for the rest of your life, or use the printer model: sell the hardware cheaply to lock you into high-priced ink refills. It&#8217;s not like Isolite Systems is exactly losing money on the mouthpieces. With this new adapter, Isolite can now make money from the dental practices that may have not been able to justify a whole-office use of their product.</p>
<p>So slippery-gripped assistants everywhere: Ding! You are now free to move about the office. Your hands have finally been relieved without a significant lightening of your employers&#8217; bank accounts.</p>

<p>So <a href="http://www.aligntech.com" target="_blank">Align Technology</a> has officially agreed to purchase <a href="http://www.cadent.biz/itero/itero.php?section=professional" target="_blank">Cadent</a>, makers of the iTero intra-oral scanning system. That means that in a few years, the only offices using impression material will be the same ones that use film and developer for x-rays. A few hours ago this email was sent out to all iTero users. I&#8217;d consider this the death knell for polyvinyl impressions:</p>

<p style="padding-left: 60px;">I am writing to share some exciting news that offers tremendous potential for Cadent and our customers. Today, Align Technology, makers of the Invisalign system, announced a definitive agreement to acquire Cadent Holdings, Inc.  Align shares our belief that intra-oral scanning is one of the most exciting technologies available to doctors. Intra-oral scanning systems provide a chair-side platform for accessing valuable digital diagnosis and treatment tools, with tremendous potential for enhancing accuracy of records, treatment efficiency, and the overall patient experience.</p>
<p style="padding-left: 60px;">While working together on interoperability and Invisalign applications to run on Cadent iTero and iOC scanners, we saw an opportunity to combine our two companies&#8217; capabilities and market-leading technology to bring innovative new diagnosis and treatment tools to our customers, thereby extending the value of intra-oral scanning in dental practices.</p>
<p style="padding-left: 60px;">After the satisfaction of all legal and regulatory requirements, Cadent will become a wholly owned subsidiary of Align Technology. They plan to keep the existing family of Cadent products, and will combine their expertise with ours to bring innovative new tools to our customers. This type of integration will take some time; while under way, each company will remain focused on its own customers and service commitments. Once the acquisition closes &#8211; which we expect in the second quarter of this year &#8211; we will have more information to share with you about the benefits of this integration for our customers.</p>
<p style="padding-left: 60px;">Finally, you may know that we have been working with Invisalign to establish interoperability, which will allow doctors to submit Cadent intra-oral, scans in place of traditional PVS impressions for Invisalign treatment. We are currently in the final stage of Beta testing and are on track to announce interoperability in the second quarter of this year.</p>
<p style="padding-left: 60px;">We believe we are only scratching surface of what we can accomplish through 3D imaging and intra-oral scanning. Once we&#8217;ve captured an accurate digital impression of a patient&#8217;s dentition, the possibilities for how to use that data in all areas of restorative, cosmetic, orthodontic and preventative treatment are endless. Both Cadent and Align see the broader potential this technology has for improving dentistry, and together, we have the resources and expertise to turn those possibilities into valuable tools for your practice and patients. <p>Is it really the end of goopy gagging? Vinyl impression material (and alginate, for that matter) will probably always have its place, but this is the first step to making digital impressions a standard part of dentistry instead of the hip fringey thing it is now.</p>

<p>Take that, Alabama! This little slap will teach you not to hand out licenses to <a href="http://www.ehow.com/how_7593421_become-dental-hygienist-alabama.html" target="_blank">dental professionals</a> without requiring students to take a national board exam first. Or even go to college first. Hey, does this mean that a dentist from Alabama will only be able to get work as a dental assistant in all the other states, like licensed dentists from other countries, until they pass the OTHER states&#8217; board exams? The good news is that now that there&#8217;s a bunch of eyes on the situation brewing at this dental college, maybe there will be a tasty intervention coming. Remember, the first step in recovery is <strong>admitting that you have a problem</strong>.</p>

<p>If not, you&#8217;re probably one of those people that <strong>like</strong> shadows, you&#8217;ll want to go back down in your burrow and sleep through the next few months. Nothing uncomfortable or awkward for you, thank you. Please leave now &#8211; bye! &#8211; because the information below will only lead to spending money on things that you don&#8217;t care about.</p>
<p>Are they gone yet? Good. Now we can talk about this shadow business. Do you remember when you first started using loupes, how they made you sit up taller, take notice of things you never saw before, and how it is now that you cannot imagine working without them?</p>
<p>About a month ago I hooked a tiny little light onto my loupes in hopes that it would keep my back straighter. I didn&#8217;t really expect any more than that. At first it was nice, the light was whiter and just about the same intensity as the incandescent overhead light. Hmm. Then I realized that I had the power turned <em>all the way down</em> on the battery pack. What happened when I began dialing the light up was nothing short of a knee-trembling-Thank-You-Jesus epiphany moment. THE SHADOWS WERE GONE! I didn&#8217;t know that there were shadows in people&#8217;s mouths before. I didn&#8217;t realize that the shadows were extremely annoying in people&#8217;s mouths. I didn&#8217;t know that you could visualize the distal of tooth numbers 1 and 16 in full spectrum light and see down into that 4 mm pocket that never gets cleaned and scoop out that little yellow goo like a miner panning ankle-deep in a river of gold.</p>
<p>When you have a light attached to your glasses, it always spotlights your focus point exactly. It goes where you go (ouch! just don&#8217;t look your patient in the eye through your loupes). It helps you save time because you&#8217;re not always reaching up and adjusting the handle of the overhead light. And patients like it when they don&#8217;t feel like they&#8217;re in an interrogation session. It&#8217;s so free and breezy above the chair without all that equipment hanging over their heads.</p>
<p>Now that I&#8217;m using a headlamp it really is like a whole new world opened up to my eyes. It&#8217;s like the first time I started using loupes all over again. It&#8217;s like. Well it&#8217;s like something I should have done forever ago. The only two things that were holding me back until now were comfort and cost. I didn&#8217;t want to pay over $600 for what is essentially a flashlight. And I didn&#8217;t want it to be inconvenient to wear.</p>
<p><strong><a href="http://www.lumadent.com"><img class="alignleft size-full wp-image-1024" src="http://www.dentalbuzz.com.php5-20.dfw1-1.websitetestlink.com/wp-content/uploads/2011/02/Lumadentlight.png" alt="" width="217" height="181" /></a></strong>After evaluating just about everything offered at the dental meeting booths, one choice has ultimately claimed its place as my new pet: the <a href="http://www.lumadent.com" target="_blank">LumaDent</a>. <p style="padding-left: 60px;"><strong>The Price</strong>. It is extremely reasonable for a niche market like dentistry. If you find it&#8217;s hard to swallow, geez, build it yourself. You&#8217;ll still need a battery pack, which you can <a href="http://www.lumadent.com/gallery2.htm" target="_blank">purchase a la carte from LumaDent</a>.</p>
<p style="padding-left: 60px;"><strong>The Package.</strong> Totally turnkey. You get a rechargeable battery pack, a charger, a hip holster, a mount made especially for your type of existing frame, an orange filter, and all sorts of things to help you control your wires. If you have problems along the way, Dr. Don will make it right until you are happy.</p>
<p style="padding-left: 60px;"><strong>The Promise.</strong> You hope that it will be comfortable. At 5 grams and a lens no larger than the size of a dime, the LumaDent weighs almost nothing. The headlight is only noticeable because of the wires. If you&#8217;re patient and work with the wires to learn how to wear them, they will be no more difficult to put on and take off than a tie. And as I stated before, the quality of the light is exceptional.</p>
<p>If you&#8217;re getting the idea that I was somehow coerced into this endorsement with money or discounted product that&#8217;s not the case here. <a href="http://www.dentalbuzz.com/about">I simply love my loupe light</a>, I love juxtapositioning Punxsutawney Phil with shadowless dentistry and I hope to inspire you to see the way you practice in a whole new&#8230;.</p>

<p>Once again the media is out to sensationalize the debate by conjuring up images of daily vomiting, years of sinus congestion, and children who can&#8217;t go an hour without a seizure or two. And once again, it is mercury that is the alleged culprit.</p>
<p>If it really were as bad as it appears for the few people that are willing to testify before the panel, don&#8217;t you think that there would be a significantly larger population of people affected by mercury poisoning?</p>
<p>Four years ago, a panel decided that further study was necessary to understand whether amalgams give off more vapors when being placed or removed versus the amount of mercury vapor produced with chewing and brushing.</p>
<p>Maybe they were on to something there! What the FDA should do is launch a study of mercury levels in the blood, urine, and body tissues of the dentists who regularly place and remove amalgams, correlating symptoms described by those afflicted with mercury toxicity with the dentists in the study. If those symptoms are consistent in the dentists who have high levels of mercury, then go from there to decide whether amalgams are truly a problem.</p>
<p>The ADA stands behind the science. And until it is refuted, they are doing the right thing by not wavering on their position.</p>

<p><strong>UPDATE &#8211; December 20th, 2010 </strong></p>
<p>Last week, an advisory panel to the US Food and Drug Administration (FDA) convened a professional panel review to again look at the safety issues associated with mercury amalgam in dentistry. A group of scientists and dental and medical professionals, lead by the International Academy of Oral Medicine and Toxicology (IAOMT), had called for FDA to reconsider its July 2009 &#8220;no risk&#8221; classification of mercury fillings. The FDA panel concluded that there are no huge scientific flaws in the agency’s 2009 finding that mercury-based dental fillings are safe for adults and children aged 6 years and older. The panel, however, recommended that the FDA look at more data, including the latest data, on the possible health risks dental amalgam poses to pregnant women and their fetuses and to young children, particularly nursing infants whose mothers have these fillings. The panel also said the FDA should consider adding warnings for these groups to the material’s product instructions.  The ADA commended the panel’s call for continued research while offering support for the FDA’s current amalgam regulation. The panel’s call for more scientific data acknowledged concerns of dental amalgam opponents who link mercury exposure to dozens of diseases ranging from autism to Alzheimer’s disease.

<p>Now this is just sad. It seems as though a perfect storm of poorly-maintained handpieces, numb lips, and inattentive dentists has come together in a flurry of reports that patients are getting burned at the dental office.</p>
<p>Really burned, not like they&#8217;re getting ripped off burned. First, second, and third degree burns. The Food and Drug Administration (FDA) is so concerned, they&#8217;ve even <a href="http://www.fda.gov/MedicalDevices/ResourcesforYou/Industry/ucm226986.htm" target="_blank">created a letter for you to send</a> to your handpiece manufacturer, since they&#8217;re not naming names as to whose products are heating up the most.</p>
<p>Most of the manufacturers already received this letter last month in response to the FDA&#8217;s new safety alert. In the meantime, they are recommending that clinicians:</p>
<ul style="padding-left: 90px;">
<li>Are vigilant about maintaining electric dental handpieces and electric oral bone-cutting handpieces according to the manufacturer&#8217;s instructions.</li>
<li>Verify with the manufacturer the appropriate routine service interval for your dental practice, based on the actual use of your electric dental handpieces or electric intraoral bone-cutting handpieces.</li>
<li>Train personnel to properly clean and maintain the electric dental handpieces or electric intraoral bone-cutting handpieces.</li>
<li>Develop a method for tracking maintenance and routine service for each dental handpiece or electric intraoral bone-cutting handpiece.</li>
<li>Examine the electric dental handpiece attachments and electric intraoral bone-cutting handpiece attachments prior to use. Do not use worn drills or burs.</li>
<li>Do not use poorly maintained electric dental handpieces or electric intraoral bone-cutting handpieces.</li>
<li>Report overheating to the manufacturer.</li>
</ul>
